Vaccination Subsidy For Scady, Rescued Cat (Aida Rozana Bt Taramizi�s)

Hi Doc,

Last November, me and my neighbour have rescue 1 male cat. He was in injuring at his leg. We bring to the the vet�to get the treatment. After like 1 week hospitalization, we decide to give him vaccine first. This cat is actually stray cat area our house. We plan to neuter this cat soon. Date vaccination is on last 16th November and this cat just bring back from clinic on last week.

I attached together his picture and vaccine sticker. I will send the original receipt and the form to you.
