Good day ! I wish to apply for a subsidy for neutering of a female dog, with details as follows:

Condition before treatment and neutering: Healthy with minor skin condition

Condition after treatment and neutering: Healthy with minor skin condition

Other details: I fostered this female dog with her 5 puppies at end of Dec 2013 just after she gave birth and kept them until pups were old enough to be weaned and adopted in early February. All pups have been successfully adopted and the mother was sent for spaying and relocated back to USJ. She was not put up for adoption because she is scared of people.
