First-Come-First-Served

Dear Applicants,

Please be informed that all subsidy claims are paid out on a first-come-first-served basis upon receipt of the following:

1. Pre- and post-op photos of the animal and detailed write-up by email.�SOFT COPY

2. Original receipt and duly filled application form by post. HARD COPY

For those who have requested subsidies but do not follow up, please be informed that your application will only be processed upon receipt of the above documentation. We also will not keep your pre-op photos for weeks/months as we clear our folders every week.

The best is to email the complete set of photos and write-up a day before you post your application form and receipt to us.

Our 2-week deadline holds and we are not able to give an extension to anyone.

Payment of subsidies are dependent on fund availability and upon our sole discretion.

CHECK LIST:

3. Name on receipt (full name) must be bank account holder�s name. �
