Spaying And Vaccination Subsidy For Bonnie, Rescued Dog (Kavitah Ganasan�s)

This is a follow-up case. This time, we have subsidised RM150 for Bonnie’s spaying and RM20 for the vaccination. This subsidy comes from all donors through our Medical Fund (RM70) and Berjaya Cares (RM100).

Dear Dr. Chan,

I would like to request for the spaying and vaccination subsidy for my rescued dog, Bonnie.

Previously, I obtained medical fees subsidy for Bonnie from you, Dr to amputate one of her ear due to maggot infection. Now she has totally recovered from the ear surgery and she is very healthy now. Unfortunately, I could not relocate her owner but I am happy to say that I have found a adopter for her.

I have spayed and vaccinated Bonnie and she is under antibiotics now. I will be sending Bonnie to�the adopter at Setapak this Saturday once she has taken all her antiobiotics.

I would like to request for the financial help for the spaying and vaccination and please see the attachments for the completed forms, receipts, vaccination card and Bonnie’s pictures.

I have read and understood your policies. I will post the completed forms and original receipts to you tomorrow via Pos Express.�I hope you could help me with�Bonnie’s spaying and vaccination fees.
