Dear Dr. Chan,
We have taken 2 stray cats, Money and Stranger, to the vet for neutering surgery on 12 January 2015 and 13 January 2015 respectively. We would like to apply for neutering subsidy. We attach the scanned copies of the forms, receipts and their before and after surgery photos. We will post the physical copy of the forms and original receipts to you soon.
