Neutering Subsidy For Titan, Rescued Male Dog (Catherine Ong Ooi Ping�s)

We have subsidised RM150 for the neutering of this dog. Catherine did not submit photos of the vaccination card; we informed her of our requirements and she decided not to claim for the vaccinations.

Dear Dr Chan,

On the 25th March, I rescued a scrawny pup with skin disease from my neighbourhood in Kota kemuning. I have named him Titan, he has completed his 3 vaccinations n I have also castrated him on 24th May 2014.
I have enclosed pic of Titan before n after castration.

I will also hand mail the form and receipt by tomorrow to your address in SS 18.

I would like to claim subsidy for the castration n vaccination of Titan. Thank you for your kind consideration.
