Vaccination Sponsorship For 5 Dogs/puppies (Lai Mei Kueen�s)

We have fully sponsored RM125 for the vaccination of these 5 dogs/puppies, ie. Market Pup, Mimi, Kelly, Kris and Jill.

Hi Dr Chan,

I would like to apply for the subsidies of the vaccinations for these puppies that we rescued.

Market pup was found lying at the entrance of �the market car park. It look sick and fearing for the

safety of the pup, �i sent her to the vet. After a few days of monitoring, the vet said she is healthy and was vaccinated.

Mimi , a stray that was chased and thrown at was also rescued before she suffer any injuries. She is a very nice and sweet lady., small size and friendly. The vet said she is 6 months old. We will send her for spaying next week.

Pls advise .

Hi Dr Chan,

Here is another set of �puppies that i wish to apply for the vaccination subsidies. We found these 3 puppies in a box dumped in front of a shop. We took them to the vet and got them vaccinated when the vet certified they are

healthy.

Can i proceed with the claim?
