Vaccination Aid For 3 Dogs In Melaka (Yeow Mie Xi’s)

Good day to u Dr Chan,

My name is Yeow Mie Xi (Michelle). I would like to apply vaccination aid for these three dogs. My office is at Kota Laksamana, Melaka. I used to feed them everyday at my working place. They are the lucky ones. Why do I say so? This is because recently many puppies died at my working area, suspect from virus attacked. To prevent them from being infected, i brought them to the vet to get their first vaccination on 18th July 2019. I will continue to care and feed them. This is my first application and I hope I fulfill all the requirements. I will post the vaccination form and vaccination receipts via Pos Express on Monday.
