Neutering Aid For 2 Dogs In Rawang (Wan Choi May’s)

Dear Dr Chan,

Be informed that I have sent this 2 dogs (2 Female) to  the vet at Kuchai Lama for neutering on 08.09.18 and now they are re-home at one house which is located at a housing estate & one factory in another area.
I want to apply for neutering aid & attach photos before & after neutering  for reference. Furthermore, completed form & original receipt will be courier to your Subang Jaya office  via  Pos Ekspress today.
