I am applying subsidy for the following stray cats under my care named:

1. Olie (estimated 2yo)

2. Ngango (estimated 1yo)

Both have undergone the surgery on April 17, 2017 at the vet’s.  Attached herewith photos of these two cats for your reference.

I will send to you by post express the original receipt of the operation together with the completed form for your further action.
