Dear Dr. Chan,

following with the application neutering aid for the mother dog previously (receipt no: 4398). please allow me to apply again for her 3 puppies (mother dog was delivered 8 puppies) was rescued on April 2017 seri kembangan near a mall.

this puppies i will help them to find a adopter as soon as possbile.

attached herewith the form and receipt for your consideration.
