Neutering Aid For 1 Dog In Gelang Patah (Nujtiya Chareansouk�s)

We have sponsored RM130 for the spaying of this dog. We thank Ms Nujtiya for her kindness in claiming RM20 less.

Dear Dr. Chan,

I have sent 1 female dog for neutering on 17/Dec/2016.

My neighbour and I managed to catch 2 female dogs at her factory area (Gelang Patah, Johor).
However, so sad one of them was found pregnant by doctor.
Thus, I would claim only 1 dog for this time.
The bill is RM 170 with GST.
Please allow me to claim RM 130 (which is RM 20 lesser).

Attached the receipt, form and photo, please let me know if i could proceed to send you the original Receipt & Form?
