Neutering Aid For 1 Dog In Ulu Tiram (Nujtiya Chareansouk�s)

We have sponsored RM130 for the spaying of this dog. We thank Ms Nujtiya for her kindness in claiming RM20 less for this dog.

Dear Dr. Chan,

I have sent 1 female dog for neutering on 08/Oct/2016.

I found this stray dog in a coffee shop at Ulu Tiram, Johor.
She is very friendly and normally taken care by the shop owner and the residents over there.
I have released her back to the coffee shop.

I would like to claim RM 130 for this case (which is RM 20 lesser).
At first i planned to bring 2 dogs for spaying but the other one found with TVT. Once she recovered then i will send her for spaying.

Please find attached the receipt, form and photos and let me know if i could proceed?
