Neutering Aid For 1 Dog In Gelang Patah (Nujtiya Chareansouk�s)

We have sponsored RM100 for the castration of this dog. We thank Ms Nujtiya for her kindness in claiming RM30 less.

Dear Dr. Chan,

I have sent 1 male dog for neutering on 02/Oct/2016.

He is a stray dog at Gelang Patah, Johor.
I found him with a big wound on his head/neck in last 2 months but now he had recovered and released back to the colony.

The total bill is RM 130 with GST and I would like to claim RM100 for this case which is RM 30 lesser.

Please find attached the receipt, form and photos.
