Dear Dr Chan,
Margie and I would like to apply for neutering subsidy for Jackie.
She is a female dog living in a building site in Bandar Utama.
We have successfully trapped her after many failed attempts .
She has undergone the operation , and we have also had her vaccinated.
She is doing very well and we have placed her in a foster home.
I have posted the form and receipt.
