Neutering Aid For 1 Cat In Johor Bahru (Sandra Winata�s)

Hello Dr Chan, I’m writing to apply for neutering aid for one female cat name Skippy. Previously she was one of the community cat in low cost housing area and cared by one old lady.  Skippy’s fortunate enough to get adopted by one of the tenant.  I brought her for spaying on 17 September and discharged on the same day and  returned to the adopter. The after picture was sent by the adopter the next day. She is doing and eating well.

I forgot to claim RM10 less in my previous application,  therefore , I would like to claim RM20 less this time.

I will send the form and receipt today.
