Dear Dr Chan,
Good day. I am Lee and this is my third application for the neutering aid for the kitty that I had brought her to spay last Saturday. Please find the information below as per requested:
Short Write-up
The kitty I brought to spay last Saturday is called BiBi. In fact she is with me about half a year since the day I adopted her (around CNY). She was found in the drainage outside my house and seek for food that time.
I brought her for the spaying right after she completed the heat cycle in order to prevent her from pregnant. I requested the doctor to do ultrasound scan first before the operation in order to ensure she is not pregnant. There was no fetus found from the scan. Hence the operation was carried out without any issue.
For the fund aid application, if it is available, I would like to apply for RM90 instead of full amount. I already put a remark in the form that I filled up.

I already posted the form and the original receipt to the given address in the form yesterday using Pos Ekspres. Looking forward for your reply and confirmation.
