Good morning!

My name is Loo Sook Yin, this is my 3^rd time applying for Neutering Subsidy, I attached here with the completed AnimalCare Neutering Subsidy form, receipt and photographs of the stray dog (Mimi) for your immediate attention.

About Mimi

The dog (Mimi) is a stray dog, I found her at the LRT construction site beside the busy main road in Bandar Kinrara 3. She had given birth to 4 puppies, I fed her and the puppies for more than a month, unfortunately 3 puppies went missing, 1 died hit by a motorcar. A week later I decided to send her to vet for the OHE surgery. She is admitted to animal hospital due to wound infection at the surgical area and will be discharged on Sunday. After she is discharged, I will put her back outside my house compound and continue to feed her. I can’t keep her with me because I already have 2 dogs. 

The original receipt and form will be posted to you after Hari Raya.
