Printed Receipts For Claims

Dear Applicants,

We wish to inform you that�with effect from today,�we require the fully itemised�printed receipt�(with your full name) when you�apply for our neutering subsidies.

If the printed receipt has no name on it, please�request that the vet writes your full name on the receipt and this must be stamped and signed�by the vet. �

If the printed receipt is not itemised, please produce a note, signed and stamped by the vet, that clearly says how much has been charged for neutering. �
