Dear Dr Chan,
I, Wong Yiew Mei,rescued a mother cat and four of her kittens at a construction side 2 weeks ago. One of the kitten was very sick but I manage to nursed it back to health.
I wish to apply for vaccination subsidy for the mother cat. I have filled in the required form and once I have your approval, I will mail it with the original receipt.
