Neutering Subsidy For Jack, Male Dog In Kangar (Nujtiya Chareansouk�s)

We have subsidised RM150 for Jack’s neutering. Nujtiya helps her mother get the community dogs neutered whenever she goes home to Kangar to visit.

Hi Dr. Chan,

Last Friday (24/10)�I have sent�1�male dog�for neutering�at Alor Setar.

His name is Jack�& he is 9 mth old.

We�plan to look around if anyone would like to adopt him once he recovered.

Bill Amount is�RM275�and I wish�I could�claim for�neutering subsidy RM150.

At first I plan to spay 3 female dogs as well�but since the doctor is not available, I have to postpone again till next�round probably�Feb ’15. Temporary my mum will keep all these�3 female dogs at home so that they won’t mix with other male dogs until I go back again.

Please find attached the photos, form�and receipt.
