We have subsidised RM25 for the 2nd vaccination of Mia.

Dear Dr Chan,

Good day.

Mia’s second vaccination is due. Took her for her�vaccination today.

Can I claim vaccination subsidy? Her 3rd kitten (Bree)is still not fully recover from flu.

So she cannot�get her vaccination today.

Now that her kittens are weaning, requested vet to do xray to�her leg.

She suspected to have this injury due to car accident.

Vet mentioned that she will need a surgery to fix the hip�to minimize risk of arthritis when she is older. Initially, vet�suspected knee patella but xray shows that it was her hip.

I really do not have the fund for the surgery right now so I may need to work towards getting the fund so that she can have her surgery as soon as possible.

Attached a collage of updated picture of her, vaccination sticker & Xrays of her leg injury.

Will submit original receipt this coming Monday.
