Ben was a stray pup and was rescued on 22/3/2014 at USJ7. He was sent to Klinik Mayo for castration on 3/6/2014 (attached receipt)

Also attached the AnimalCare neutering subsidy forms and his pictures for your attention

For your information and record purposes, both 1st and 2nd vaccination for Ben were subsidised by AnimalCare
