We have subsidised RM25 for the 1st vaccination of Sim-Sim.

Now is time for her 1st vaccination before can open for adoption.
I would like to apply vaccination subsidy for Sim-Sim.

Files attached are the vaccination card with sticker.
And for your information, normally those cats for adoption, I using pencil to write their name on the vaccination record card, not using ink pen.

Because the adopter might change the cat’s name when adopted.
