Spaying Subsidy For Dolly, Rescued Female Dog (Yap Mei Han�s)

Dear Dr Chan,

Greetings.� I hope you are well.

I would like to request for your kind consideration to subsidise the neutering of a female dog�named Dolly under my care.

Dolly was rescued in July 2013.� She was around 10 weeks old at that time.� I took her in as she was too young to fend for herself and she suffered mild skin problem.�� I have tried to rehome her however no interested adopter come forward for her.

I have recently spayed her as she has reached her maturity of�7 months.

I have read�the terms &�conditions as published in animalcare website and confirmed my acceptance to all the requirement as stated there.��The fee of RM250 was fully paid by me and�I didn’t receive any donation from public.

I enclose the photo of Dolly taken at the vet and a copy of the bill.
