Vaccination Sponsorship For 11 Puppies (Kimberly Ong Bee Kin�s)

This is a follow-up (2nd vaccination and deworming). We have fully sponsored RM275 for the 2nd vaccination of these 11 puppies and subsidised RM16.50 for their deworming. This total subsidy of RM291.50 comes from the AnimalCare Donors (RM91.50) and Berjaya Cares (RM200).

Dear Dr Chan,

I, Ong Bee Kin (Kimberly) would like to submit claim 2nd vaccination for 11 puppies which I claimed for 1st vaccination before.�Previously 12 puppies, the photo of black puppy with the new owner sent (1 got adopted).

Will be bring this 11 puppies to adoption drive coming weekends, hopefully they will have a chance to find a good home.
