The bill for the 4 puppies is RM44 and we have subsidised RM22 for it.
The bill for Pooch is RM35 and we have subsidised RM25 for the vaccination and RM2.50 for the deworming.
This RM49.50 subsidy comes from the AnimalCare Donors (RM19.50) and Berjaya Cares (RM30).

Subject: Request for subsidy for medical treatment of 4 pups for Isospora infection

I would like to make another application for medical treatment of 4 young black pups approximately 3 weaks old. The puppies were picked up at a factory lot area at USJ1. There were initially 6 puppies, all whom initially seemed healthy and active. However, after a few days, two of the puppies died, one on 13th of November, and another one two days later on the 15th. It was determined that they had Isospora intestinal infection. The remaining four puppies were sent for immediate checkup on the 15th and as of now, there is no change in their appearance before and after the treatment.
The claimable amount paid is as follows:
Scourbun stanzil: RM36.00
Sulpheldiazine: RM8.00
Any amount which could be subsized is greatly appreciated.

Subject: Medical Claim for vaccination and antidiarrhea treatment of male puppy

I would like to apply for medical subsidy for the vaccination and antidiarrhea treatment of one male puppy named Pooch which my mother found slightly over a month ago with severe mange which is now almost fully healed. The puppy is active and mostly healthy before and after vaccination except for the slight diarrhea, so there is no significant difference in before and after photos. Attached the copies of the bill, medical subsidy form and current photos of the puppy.
Amount paid was as follows:
Antidiarrhea: RM5
Vaccination: RM30
TOTAL: RM30
