Medical Subsidy For 5 Rescued Puppies (Yap Mei Han�s)

I� hope you are well.� It has been a while.

A pregnant dog nearby my house recently delivered 5 puppies in a drain and I�retrieved�them out�on 02 November 2013.� That area�is�a common place where the council catches the stray.� I was concerned�of their safety.

I sent them to the vet for vaccination, dewormed and medical treatment.� I would like to request for subsidy from animalcare on the following :
a) Vaccination of 5 puppies
b) deworming of 5 puppies
c) Medical treatment for 1 puppy

All the 5 puppies received their first vaccination and was dewormed. One of the puppy requires extra medical treatment as his gum is�pale and slightly malnourished.

I enclose the photos of their living condition in the drain, their individual pictures,�vaccination card and the receipts (for details breakdown).

I have read and understood the policies of the subsidy set by Animal Care.� I confirm the bills was fully paid by myself with no donation received from other sources.
