Medical Subsidy For Dog With Neck Injury (Carol Ng Siew Theng�s)

Dear Dr. Chan,

I wish to apply for subsidy for medical treatment of a black male stray dog which I found near my office area with a deep neck wound and various mild wounds like bite marks on his body, most likely due to a dog fight.The dog was boarded at at the clinic on 17 October until 22 October. On day of discharge on 22nd Oct, the neck wound was dry and no longer open, although evidence of the neck and other minor bodily wounds are still visible. The dog is otherwise active and in good condition.

Amount paid as per bill:

1) Consultation = RM25
2) Lab test = RM30

3) Boarding = RM75

4) Oct/Med = RM50

5) Oct/Med = RM15
