One of our readers from Kuantan suggested that Maslina bring Obot for a second opinion at her vet’s, so that was done, and here is the latest news:

Hi Kah Yein,
Maklumat terbaru Obot. Saya telah membawa obot ke klinik kedua seperti yang dicadangkan oleh saudari (reader AnimalCare) semalam 30/4/2013.. Amat memeranjatkan. Dr yang melihat keadaan Obot memberitahu saya Obot tidak menghidap pyometra. Berbanding dengan dr pertama yang menggunakan x-ray, Dr ini hanya mengambil darah dan melihatnya di bawah microscope.
Menurut dr ini, jika obot menghidap Pyometra keadaanya sepatutnya tidak mahu makan, letih dan muram.. berbeza dengan obot yang banyak makan. Saya juga ada bertanya berkenaan perutnya yang besar.. Dr memberitahu kemungkinan ia keadaanya dari dulu… Melihat keadaan sporo obot pula, Dr ini memberitahu keadaanya juga telah baik tetapi masih perlu makan ubat.. Keadaanya ini jugamembolehkanObot untuk dikasi..
Saya tidak tahu apa yang perlu saya lakukan.. perbezaan pendapat antara 2 doktor.. Oleh itu saya membuat keputusan mengikut apa yang Dr ini cadangkan.. Obot telah dikasi semalam..
Keputusan pagi ini,
1) Obot tidak makan banyak seperti selalunya – pendapat saya mungkin lepas pembedahan, selera makan kurang
2) seperti ada kesan seperti cairan nanah tetapi tidak banyak dan sedikit kering di tempat pembedahan..
Saya akan cuba memantau keadaan obot..Amat berharap keputusan yang telah dicadangkan oleh Dr ini tepat..

I was thrilled to know that Obot has no pyometra as that’s a very severe disease which requires no delay in surgery (in general). But I was puzzled as to why there is a difference in diagnosis between the two vets so I consulted a vet for an opinion.

Here is a possible explanation (please note that this is just a hypothesis based on the information we have and the purpose of sharing is just to…share and learn):

It is possible that Obot did have pyometra but during the course of treatment for sporotrichosis, the antibiotic used could have reduced the pyometra (which is pus in the uterus due to an infection, a potentially fatal condition if not treated). After having undergone the course of antibiotics, the pyometra is reduced to a state where it no longer shows up in the blood test (done by the 2nd vet).

Please also note that X-Ray, ultrasound or blood test do not determine conclusively that pyometra is present. All three tests are not 100% reliable.

Hence, this explains why the pyometra cannot be detected now.

And the best news is that spaying would take care of the pyometra for good, in the event that it was there in the first place, which means, Obot is free from pyometra now!

However, the treatment for sporotrichosis has to continue.

Good news, isn’t it?

So it sometimes pays to have a second opinion. Not that the first opinion was not good enough, it is just helpful to look at the case with a fresh pair of eyes. As a further note, I know of vets who actually encourage patients to be taken for a second opinion so that we can be more certain about the correctness of a diagnosis and also widen the options of treatment for the patient. I hope we will seek a second opinion with this purpose in mind, especially for difficult cases.
