Dear Dr. Chan,

A very good day to you.I am writing to you regarding the accident cat that underwent surgery at the vet’s clinic. On, 22 October the cat was brought back to the vet for follow-up treatment. The wire used to hold the broken-jaw together, had since been removed. At the same time, I have also requested the vet to neuter it.

The cat has now recovered 90% and is no longer under medical care. I have�release it back to the community.It will return to my backyard for food twice a day.

I hereby, would like to apply for subsidy for neutering the cat.

Attached, please find a copies of the receipt; the application form�and latest photo of the cat.
